The drug and alcohol assertive outreach service’s main aim is to significantly improve the re-engagement of clients who are dropping out of Horizon treatment services. It also aims to promote the service to those who may need it. Horizon clients drop out of treatment or don’t access treatment for all sorts of reasons; missed appointments, lack of motivation, illness, family or relation problems, treatment naivety etc. and the team will help these individuals get into treatment or back into treatment if things have gone wrong. The team also help people who are most at risk of acute illness and early death, such as chaotic drug and alcohol users and the homeless and will work with clients in a variety of settings such as hospitals, soup kitchens, homeless drop-ins and including the clients home to ensure maximum engagement and re-engagement with drug and alcohol treatment services and the promotion of recovery. This is also a harm reduction focused service whereby the outreach workers will use a range of initiatives to reduce harm to the individual, their families and the community. 
Your main responsibilities involve:

  • Meet the Outreach Workers in the main office, on time to discuss and assist in planning the day’s activities and referrals
  • Assist the Outreach Worker in their duties by acting as their helper
  • Accompany an Outreach Worker to external visits to hospitals, Mental Health facilities, homeless drop in centres, soup kitchens and people’s homes
  • Accompany the Outreach Worker to other services within Horizon to take referrals and book appointments
  • Take direction and work within professional boundaries
  • Give harm reduction advice when appropriate
  • Work with the Outreach Worker to ensure all visits are risk assessed
  • Answer general queries about the service
  • Report any untoward incidents
  • Keep all client information confidential
  • Attend relevant training sessions
  • Be professional and act as an ambassador to the service
  • Promote the organisation and its mission
  • Demonstrate the organisations values and principals in your actions and behaviors
